Refer a Patient
Our sleep physicians will diagnose problems and offer effective solutions that will not only help patients with their sleeping disorder, but also improve their quality of life. To assist you in providing follow-up care, we will make sure that you receive updates and information on their treatment here at the Sleep Disorders Center of East Alabama.
Self-Referral
Although most of our patients are referred to us by physicians, there are times when a patient decides to make a self-referral. As with all patients, our physicians must have certain information to provide you a thorough and accurate medical opinion. This information may include:
- Physician office notes
- Pathology reports
- Radiology reports
- Surgical reports
- Correspondence records
To make a self-referral, call 334-528-2404. A staff member will let you know what documents to bring with you to the Sleep Disorders Center. Those items will be reviewed by the physician, and an appointment will be made.
Please note that some insurance plans require that a referral be made only by a physician. Each patient should review his or her individual policy.
